A standard eye chart can show whether your vision has changed. It cannot show every small change happening beneath the surface of your eye. Optical coherence tomography, often called OCT, gives your optometrist a highly detailed cross-section view of important eye structures, helping to identify concerns that may not yet be affecting your sight.

What optical coherence tomography actually does
OCT is a non-invasive imaging scan that uses light to create detailed images of the retina, macula and optic nerve. These are delicate structures at the back of the eye that are essential for clear, comfortable vision.
Think of it as looking at the eye in layers rather than only viewing its surface. During a routine retinal examination, your optometrist can inspect the back of your eye directly. An OCT scan adds depth, allowing them to measure the thickness and shape of retinal layers and compare those measurements over time.
The scan is quick, painless and does not touch your eye. You simply rest your chin on the instrument, look towards a target and keep still for a few seconds. Most people find the process straightforward. In many cases, drops are not needed, although your optometrist may recommend dilation if a more comprehensive retinal assessment is required.
Why this detailed scan matters
Some eye conditions develop gradually and quietly. By the time a person notices a significant change to their vision, damage may already have occurred. OCT does not replace a full eye examination, but it helps your optometrist detect subtle structural changes early and monitor them with greater confidence.

Checking the optic nerve for glaucoma risk
Glaucoma is often associated with increased pressure inside the eye, but eye pressure alone does not diagnose it. Some people have glaucoma with pressures in the normal range, while others have higher pressure without evidence of damage.
OCT can measure the nerve fibre layer around the optic nerve. Thinning in this area may be associated with glaucoma or a heightened risk of developing it. Your optometrist will interpret the scan alongside your eye pressure, family history, visual field results, optic nerve appearance and overall health.
This matters for people with a family history of glaucoma, those over 40, patients with diabetes, people who have used steroid medication for an extended period, and anyone previously told they have high eye pressure or suspicious-looking optic nerves. It also explains why regular eye examinations are worthwhile even when your vision seems perfectly clear.
Assessing the macula and central vision
The macula is the central part of the retina responsible for the detailed vision used for reading, recognising faces, using a mobile and seeing fine print. OCT can identify changes in and around the macula, including swelling, fluid, traction and thinning.
Macular scans can be particularly helpful when someone notices wavy lines, a blurry patch in the centre of their vision, difficulty reading despite updated glasses, or a sudden difference between the two eyes. These symptoms should not be ignored, especially if they begin suddenly.
OCT may also assist in monitoring retinal effects related to diabetes and other medical conditions. If a scan raises concern, timely referral and communication with your GP or eye specialist can help ensure you receive the right care without delay.
Investigating unexplained changes in sight
Sometimes a patient has symptoms that do not match what an eye chart or glasses prescription alone would suggest. They may describe reduced contrast, a shadow, intermittent blur or vision that is not as sharp as expected after updating their spectacles.
In these situations, OCT gives another layer of information. It can help distinguish whether symptoms may be related to the retina or optic nerve, or whether another cause such as dry eye, cataract, prescription change or a corneal issue should be considered. It is one reason a medically focused eye examination goes beyond simply checking whether you can read the smallest letters.
What to expect during an OCT appointment
Your optometrist will first talk through any concerns you have, including changes to your vision, headaches, eye discomfort, flashes, floaters, medical conditions and family eye history. They will then perform the tests that are appropriate for your needs. OCT may be included as part of a comprehensive assessment or recommended because of a particular finding.
The scan itself usually takes only minutes. You will be asked to look at a small fixation light while the machine captures images. There is no radiation, no injection and no recovery time. You can generally return to work, school or your usual activities straight afterwards.
The more important part is the interpretation. OCT images contain a great deal of information, and normal anatomy can vary from one person to another. Your optometrist will explain what the scan shows in plain language and discuss whether follow-up imaging is recommended.
For children, an OCT scan may be useful in selected circumstances, particularly where there is concern about the optic nerve, reduced vision that needs further investigation or a relevant family history. Whether a scan is practical depends on the child’s age, comfort and ability to hold still briefly. A patient, family-centred approach makes a real difference here.
An OCT scan is valuable, but not a diagnosis on its own
Advanced equipment is helpful only when it is considered alongside the person in the chair. An OCT result should never be viewed in isolation. A scan may show an unusual measurement that is normal for that individual, or it may appear normal despite symptoms that need further investigation.
That is why your optometrist may combine OCT with visual field testing, retinal photography, eye pressure checks, a slit-lamp examination and a detailed health history. If required, they can also refer you to an ophthalmologist for specialised testing or treatment.
There are practical limits too. Poor tear film quality, blink-related movement, cataract and difficulty maintaining fixation can affect scan quality. This does not mean the test has failed, but it may mean repeating the scan or using other examination methods. For patients with dry eye, treating the ocular surface can sometimes improve both comfort and the reliability of imaging.
When should you book an eye examination?
An OCT assessment may be appropriate if you have a known eye condition, a family history of glaucoma or macular disease, diabetes, unexplained changes to your vision, or concerns raised at a previous examination. It can also be a useful part of ongoing monitoring where your optometrist has identified a risk factor.
Seek urgent eye care if you experience a sudden curtain or shadow in your vision, new flashes with a shower of floaters, sudden loss of vision, marked distortion, severe eye pain, or a red painful eye with blurred sight. These symptoms need prompt assessment and should not wait for a routine appointment.
